Sparrows Player Waiver Agreement

Sparrows Volleyball is an affiliated club of Volleyball NSW and hence covered by Volleyball NSW insurance.

To be covered by Volleyball NSW (VNSW) insurance, players must pay a membership fee (Full $115 and Recreational $55 for adults OR FULL $85 and Recreational $40 for under 18's). Membership commences each year on 1st April and expires on 31st March the following year. Learn more. Full membership is for SVL players otherwise recreational is fine.
ALL Participants must complete this activity waiver form and where the applicant is under 18 years of age, this form must also be signed by the applicant’s parent or legal guardian. 
Players who choose not register on our Revolutionise SPORT portal can play but accept they will not be covered by our insurance. If you play Sydney Volleyball League (SVL) or other clubs, you will most likely already have paid for the membership and will only need to add Sparrows Volleyball as a club through link below.

Player Waiver Form

Details of Activity: The Participant will be participating in volleyball activities (the "Activity") provided by Sparrows Volleyball (the "Activity Provider"). 

Exclusion of Liability: Except where provided or required by law and such cannot be excluded, I (the Participant) agree that it is a term of my participation in activities delivered by Sparrows Volleyball, its directors, servants and agents including all coaching and management staff, is absolved from all liability however arising from injury or damage however caused (whether fatal or otherwise) arising out of my membership and/or participation in any Sparrows activity including but not limited to matches or training. The Participant understand that the Participant would not be permitted to participate in the Activity unless the Participant signed this Waiver. 

Participants have voluntarily read and understood this warning and accept and assume the inherent risks in volleyball & understand that although Sparrows Volleyball and its staff and coach(s) and service providers attempt to minimise any risk of personal injury within practical boundaries, accidents do happen and all physical activities carry the risk of personal injury.  

Release and Indemnity: In consideration of the provider permitting me to participate in Volleyball activities and programs, I: (a) release and forever discharge the provider from all claims that I may have or may have had but for this release arising from or in connection with my membership and/or participation in any activity provider activities and programs; and (b) indemnify and hold harmless the activity provider, to the extent permitted by law in respect of any claim by any person including but not only another member of activity provider, arising as a result of or in connection with my membership and/or participation in any activity provider activities or programs.  

Fitness to Participate: I declare that I am and must continue to be medically and physically fit and able to participate in any Sparrows Volleyball activity and program. I am not and must not be a danger to myself or to the health and safety of others. I will immediately notify Sparrows Volleyball, in writing of any change to my fitness and ability to participate. I understand and accept that Sparrows Volleyball and its staff and coach(s) will continue to rely upon this declaration as evidence of my fitness and ability to participate.   

Participants have read, understood, acknowledge and agree to the above declaration including the warning, exclusion of liability, release and indemnity and fitness to participate.  

Consent Form

Participants/parents/guardians hereby consent to the Sparrows Volleyball to take or have taken by others, photographs, digital images and/or audio and/or video footage of the participant named below, and to store the images, make copies of the images and publish the images in any form, in whole or in part, and distribute them in any medium. Participants/ parents/ guardians consent to the images being used by the provider or provided to others for promotion to help them achieve their vision and consent to receiving to marketing and communication to stay informed. Participants understand that there will be no payment for giving this permission and hereby waive any claim that I or we may have or may have had for remuneration, residuals, royalties or any other payment in respect of use of the images. I agree that Sparrows Volleyball shall not be bound to make any use of the images and consent to receive communication from Sparrows Volleyball. If you do not have photo consent and let the organisers know on the day.
